Localities: DESMOND is currently being delivered throughout Kent and Medway.

Local Referral Pathway: Patients need to be referred through their GPs to Spirit Hub. We ask GPs to provide us with up to date bloods results.

Modules Delivered:

We offer face to face desmond for anyone who wishes to take the course.

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)
