Localities: DESMOND is currently being delivered throughout Southwark and we deliver at 3 sites; Tessa Jowell Health Centre ( Dulwich), Bermondsey Health Centre; Diabetes Centre, Guys and St Thomas’ Hospital
Local Referral Pathway: Referrals received from GP Practice, HCP and Self Referral via the Diabetes Book & Learn Website

Newly Diagnosed module available (for patients up to 12 months post diagnosis)
