Local Referral Pathway: Referrals received from GP Practice, Practice Nurse, Community Diabetes team, Hospital Diabetes team and Self Referral

Virtual DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

Virtual DESMOND Foundation module available (for those with established diabetes)

Overview

Unfortunately due to COVID-19 situation all of our face-to-face group sessions have been suspended.  We currently are offering the programmes below.

DESMOND Newly Diagnosed programme – Virtual Group

  • Delivered via an online group chat platform called Zoom

  • Delivered by trained Educators

  • For groups of up to 10 people with Type 2 diabetes

  • Delivered over 2 x 3 hour sessions, 1 week apart

MyDESMOND programme

  • Interactive, educational material that mirrors the DESMOND face-to-face groups

  • 8 weekly educational booster sessions

  • Activity level, weight and healthy eating tracking options

  • Ask the Experts – specialist team available to answer your questions

  • Chat – users have opportunity to connect and support each other through the chat forum.
