Newly Diagnosed module available (for patients up to 12 months post diagnosis) virtually

DESMOND Foundation module available (for those with established diabetes) virtually

Overview

Who are we?

Welcome! We are the South Eastern Health and Social Care Trust DESMOND team. Our experienced team of educators come from a variety of nursing and dietetic backgrounds.

As a service we are based at the Lagan Valley Hospital, Lisburn, however we deliver the DESMOND programme right across the South Eastern Trust patch; from North Down and Ards, to Downpatrick and Newcastle.

What do we offer?

We normally deliver face-to-face sessions in Lisburn, Bangor, Ards and Castlewellan. In addition, we are now pleased to offer Virtual DESMOND using ZOOM from the comfort of your own home. We are also able to set you up with your own MyDESMOND online account.

We offer our DESMOND courses as x 2 three hour sessions, held one week apart. Courses are held Monday to Friday from either 09:30 to 12:30 or 13:30 to 16:30.

How can you get involved?

If you have been diagnosed with Type 2 Diabetes, are registered to a GP within our Trust area, are over the age of 18 and not currently treating your Type 2 Diabetes with insulin, you are eligible for DESMOND!

We can accept referrals from your GP, Diabetes Specialist Nurse, Practice Nurse, Dietitian or healthcare professional. You will then be placed on our waiting list and contacted when we have a place for you.
