Localities: DESMOND is currently being delivered throughout Dublin North, Dublin North City and Dublin North West area

Local Referral Pathway: Referrals received from GP Practice, Practice Nurse, Diabetes Nurse Specialist and Self Referral. Self Referrals can be made through the HSE link for Diabetes courses or directly to Amanda Dornan, Administrator

Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)

DESMOND is a 6 hour programme delivered over one full day or two half days. Face-to-face group programmes are paused at present. Currently running Virtual DESMOND programmes in the area.
