Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)

2 x 3 hour Virtual Sessions

Face-to-face when we are able to resume

Overview

The DESMOND programme in Camden is available to all those registered with a Camden GP. We accept referral from your GP or alternatively you can self-refer by contacting us via telephone on 0203 758 2036 or email at diabetes.ckd@nhs.net. Once we have accepted the referral, you will receive available session dates for you to choose from. Please contact us to book on your chosen session.
