Modules Delivered:

DESMOND Newly Diagnosed module available (for patients up to 12 months post diagnosis)

DESMOND Foundation module available (for those with established diabetes)

DESMOND is delivered virtually – via MS Teams Mondays, Wednesdays, Thursdays and Saturdays and Face-to-face. We are now providing 50% Face to Face and 50% virtual.
